Closed or blocked register. The simplest cause, and we always rule it out first. A register damped shut, painted over, or blocked by furniture in a rarely used room reads exactly like a duct fault. We check it before opening any access panel.
The room fed by a heat pump that is underperforming. On a property running a heat pump that is low on charge or stuck in a defrost fault, a room fed by that unit can lag behind the rest of the house even with sound ductwork.
